This Liability Agreement (“Agreement”) is made between SOS Dance Studio (“Studio”) and the undersigned participant (or legal guardian, if applicable), hereinafter referred to as “Participant.” By signing this Agreement, Participant acknowledges and agrees to the following terms:

1. GENERAL LIABILITY WAIVER

Release of Liability and Assumption of Risk Participant understands and acknowledges that participation in dance classes, rehearsals, or performances involves physical activity that may result in injury. Participant voluntarily assumes all risks associated with participation and agrees to release, waive, and hold harmless SOS Dance Studio, its instructors, employees, and agents from any and all claims, liabilities, damages, or demands arising from participation.

2. PHOTO AND VIDEO RELEASE

Permission to Use Likeness Participant grants permission to SOS Dance Studio to photograph and record video during classes, performances, and events. These images may be used for promotional purposes, including but not limited to social media, website content, and printed materials. Participant waives any rights to royalties or compensation for their use.

3. MEDICAL WAIVER

Emergency Medical Treatment Authorization In the event of an injury or medical emergency, Participant authorizes SOS Dance Studio to seek appropriate medical treatment. Participant understands that they are responsible for any medical expenses incurred.

4. COVID-19 WAIVER

Assumption of Risk and Waiver of Liability Related to COVID-19 Participant acknowledges the contagious nature of COVID-19 and voluntarily assumes the risk of exposure or infection through participation in activities at SOS Dance Studio. Participant agrees to hold harmless the Studio, its staff, and affiliates from any claims arising from exposure or infection.

5. CANCELLATION AND RESCHEDULING POLICY

• Cancellations within 6 hours of the scheduled class are not permitted, and no refund will be issued.

• If Participant needs to reschedule, it must be done at least 6 hours before the class start time.

• Failure to reschedule within the appropriate timeframe will result in the forfeiture of payment for the missed class.

• To reschedule, Participant must call, text, or email SOS Dance Studio.

6. CODE OF CONDUCT

To maintain a positive and respectful environment, all participants must:

• Show respect towards instructors, staff, and fellow students.

• Wear appropriate dance attire and follow the studio’s dress code.

• Arrive on time and be prepared for class.

• Avoid disruptive behavior that interferes with the learning experience. Failure to adhere to these guidelines may result in dismissal from the Studio without a refund.

7. INJURY REPORTING POLICY

Any injuries sustained during class must be reported to the instructor or Studio staff immediately to ensure proper care and documentation.

8. PERSONAL BELONGINGS DISCLAIMER

SOS Dance Studio is not responsible for any lost, stolen, or damaged personal belongings. Participants are encouraged to keep valuables at home or secure them in designated areas.

9. PAYMENT POLICY

• All class payments are non-refundable except as stated in the cancellation and rescheduling policy.

• Any outstanding balances must be paid before participation in future classes.

• Late payments may be subject to additional fees.

10. DRESS CODE POLICY

To ensure safety and comfort, all participants must adhere to the following dress code:

• Proper dance shoes appropriate for the class style.

• Comfortable, form-fitting clothing that allows for movement.

• No jewelry that could cause injury. Failure to follow the dress code may result in being unable to participate in class.

11. LIABILITY FOR PROPERTY DAMAGE

Participants (or parents/guardians) are responsible for any damage they cause to Studio property due to negligence or failure to follow instructions. A fee may be charged for repairs or replacements.

12. EMERGENCY CONTACT INFORMATION

Participant must provide an emergency contact name and phone number before participating in any class.

13. INCLEMENT WEATHER & STUDIO CLOSURE POLICY

In the event of class cancellations due to inclement weather or Studio-related closures:

• Participants will be notified via email, text, or phone call.

• Refunds or rescheduling options may be provided at the Studio’s discretion.

14. CONSENT FOR PHYSICAL CONTACT

Participants acknowledge that dance instruction may require physical corrections by instructors for guidance and safety. If a participant does not consent to this, they must notify the instructor before class.

15. GUEST & SPECTATOR POLICY

• Only registered students may enter the dance floor during class.

• Guests or spectators are allowed only in designated areas.

• Unauthorized guests may be asked to leave.

16. DISPUTE RESOLUTION CLAUSE

In the event of a dispute, Participant agrees to resolve any claims through mediation or arbitration before pursuing legal action against SOS Dance Studio.

17. LATE ARRIVAL POLICY

Participants arriving more than 30 minutes late may not be allowed to join the class for safety reasons.
